These internal hemorrhoids occasionally bleed, but don’t extend outside of the rectum.

At this level, hemorrhoids may prolapse or extend out of the anus.

Level 3 hemorrhoids usually bleed and cause pain. They may prolapse when put under strain.

These cause pain, bleeding, discharge, and possible tissue damage.

An inguinal hernia in particular can cause testicle pain (and swelling).

You might experience sharp or burning sensation (especially with movement or lifting).
